Case Study:

Patient: Sarah Thompson, 28-year-old woman

Presenting Complaint: Excessive hair growth on face, chest, abdomen, and upper arms for the past 5 years. Significant emotional distress and social anxiety affecting personal and professional life.

Medical History: Regular menstrual cycle, no history of weight gain, occasional acne, no family history of PCOS or other hormonal conditions.

Physical Examination: Moderate hirsutism on face (Ferriman-Gallwey score of 12), chest, abdomen, and upper arms. No other significant physical findings.

Laboratory Investigations:

  • Elevated serum testosterone levels (160 ng/dL)
  • Normal LH, FSH, and prolactin levels
  • Normal thyroid function tests
  • Abdominal ultrasound revealing multiple small cysts in both ovaries

Diagnosis: Hirsutism associated with Polycystic Ovary Syndrome (PCOS)

Treatment Plan:

  • Lifestyle modifications: Weight management through healthy diet and regular exercise, stress reduction techniques like yoga and meditation.

  • Medication: Metformin to manage insulin resistance and regulate menstrual cycles, spironolactone to lower testosterone levels and suppress hair growth.

  • Laser hair removal for long-term hair reduction on face and other visible areas.

  • At 3 months: Sarah reports improved energy levels, more regular menstrual cycles, and reduced acne. Hirsutism score decreased by 2 points.

  • At 6 months: Hirsutism score further reduced by 4 points, showing significant improvement in unwanted hair growth. Sarah expresses increased confidence and improved social interactions.

  • At 1 year: Sarah maintains stable weight, regular menstrual cycles, and minimal visible hair growth. She actively participates in social activities and reports regained confidence in her personal and professional life.
